Community Living Supports
Michelle P. Waiver
Community Living Supports (CLS) services are provided for children and adults through the Michelle P. Waiver and facilitate independence and promote community participation. This service is designed to allow individuals residing in their own home or their family’s home to have fun in life and learn skills that connect them to helpful resources.
CLS services are provided by a Direct Support Professional (DSP).
ABI Long Term Care Waiver
CLS services are provided for adults through the ABI Long Term Care Waiver to facilitate independence and promote community participation. This service is designed to allow individuals residing in their own home or their family’s home to have fun in life and learn skills that connect them to natural resources. Community Living Supports are provided as specified in the individual’s plan of care and may include the following:
- Daily assistance
- Activity training
- Laundry
- Routine household care and maintenance
- Activities of daily living
- Personal hygiene
- Shopping
- Use of money
- Medication management
- Socialization
- Relationship building
- Leisure choices
- Participation in activities
- Producing therapeutic outcomes
CLS services are provided by a Direct Support Professional (DSP).
